مقدمه:یکی از اهداف سیستم­ های سلامت، کاهش نابرابری در دسترسی به مراقبت ­ها و ارتقاء سلامت مردم می­باشد. تمرکززدایی در چهار دهه گذشته با هدف بهبود فرایندهای مدیریتی و ارائه خدمات مناسب مطرح بوده است. هدف مطالعه حاضر بررسی پیامدهای تمرکززدایی با به­ روزرسانی مطالعات مرور منظم می­ باشد.
مواد و روش­ ها: این پژوهش حاصل به­ روز­رسانی آخرین مطالعات مرور منظم تمرکززدایی سیستم سلامت (مارس2016) می­ باشد. برای این مطالعه تمام مقالات پژوهشی اصیل تمرکززدایی سیستم سلامت با جستجو در پایگاه های اطلاعاتی معتبر جمع ­آوری شدند و پس از حذف موارد تکراری و غیرمرتبط، ارزیابی کیفی شدند.
یافته ­ها: از 640 مقاله یافت شده هفت مقاله معیارهای ورود را داشتند. تمرکززدایی در کشورهایی با ساختار سازمانی و ظرفیت محلی مناسب باعث بهبود حاکمیت و مشارکت فعال جامعه، اعتماد به مراقبت­ های غیرمتمرکز، کاهش فاصله سفر، افزایش دسترسی به درمان، دستیابی به پوشش همگانی و بهبود همه پارامترهای مهم ارائه خدمات از جمله کیفیت شده. همچنین باعث افزایش ویزیت سالانه و کاهش مرگ و میر نوزادان شده که می­تواند عدالت در دسترسی و پیامدهای سلامت را ارتقاء دهد.
نتیجه­ گیری: تمرکززدایی فرصتی برای مشارکت جامعه و کارکنان در برنامه ­ریزی، بودجه­ بندی و فراهم شدن زمینه عدالت در تخصیص منابع و در نتیجه باعث ایجاد انگیزه و بهبود کیفیت ارائه خدمات شده. با توجه به این که اجرای تمرکززدایی در هر کشوری بستگی به رهبری سیاسی، زمینه­ های اجتماعی- اقتصادی، زیرساخت­ های قبل از اجرا و مهارت­ های مدیریتی و فنی در سطح محلی دارد، بنابراین، باید عوامل کلیدی سیاست­گذاری بخش سلامت در طراحی و پیاده ­سازی استراتژی ­های فنی، درک کامل از زمینه آن کشور داشته باشند.
