Introduction: Integration is a type of structural change which is performed with the target of concentrating on decision-making, resource saving and outsourcing reduction. Organizational diagnosis, as an introduction to the development and reconstruction of an organization, is a systematic process of collecting data to solve its problems, challenges and restrictions. This study was conducted in order to evaluate the results of the integration of the medical health care system with higher staff in a healthcare organization.

Materials and Methods: This qualitative study was carried out in a healthcare organization. Samples consisted of 18 top and senior managers whom were purposefully selected. The qualitative data of   the semi-structured interviews were analyzed with content analysis method using MAXQDA10 software. The validity of analysis was confirmed with external validity, participant’s assessment and evaluation of interviewers up to 85% and the reliability was confirmed by re-encoding up to 90%.

Results: Integration had various strengths and weaknesses in four fields of concentration, mission, coordination and effectiveness. It actually lead to shrink the administrative structure and increase efficiency, but at the same time failed to gain reasonable effectiveness. The integration of staff and line weakned medical education. About 67% of managers disagreed to continue the integration.

Conclusion: It is nessecery to carefully check the problems of the current situation and set goals before integrations. Receiving managers’ viewpoints in different parts of organizations would definitely help to achieve integration goals.
